Arriving for an office visit
Come to the 3650 Professional Building
Our office is in Suite 205B. If you are scheduled for a procedure at the hospital, use the arrival instructions from the hospital instead.

What to bring to your first visit
Medical history
Bring your medication and allergy list. Be ready to discuss your symptoms, previous treatment and medical history.
Records
Bring relevant reports or have them sent before the visit. Let us know where any original imaging can be viewed.

Write down your questions so you can discuss them at the visit.

Before leaving home
- Bring a photo ID and current insurance information.
- Bring a complete medication and allergy list.
- Follow only the preparation instructions given for this appointment.
- Allow extra time for parking and finding Suite 205B.
- Tell the office in advance if you need an accommodation or communication assistance.
