Reason for referral, working diagnosis, urgency, and the specialty requested.

Relevant note, operative history, laboratory results, pathology, and imaging reports. Include where images can be accessed.

Confirm patient details
Demographics, insurance information if applicable, and reliable contact information.

Call for time-sensitive concerns

Surgery and anesthesia
Preoperative and anesthesia referrals
For pre-op or anesthesia review, include the planned procedure, current medications, relevant cardiac or pulmonary history and any previous airway or anesthesia problems.

Send office referrals to Virginia Surgical Services in Suite 205B. For procedures at Inova Fair Oaks Hospital, the hospital provides separate scheduling and arrival instructions.
